Uranium Exposure and Contamination:
Preliminary Self-Assessment Questionnaire
The Preliminary Self-Assessment Questionnaire is presented for use by civilians, NGO staff and military personnel who live in or who have served in theatres of war and where bombs and munitions are suspected to contain uranium alloys.
This checklist is a preliminary self-assessment tool for individuals who have reason to suspect they may have been contaminated by uranium as a result of exposure to uranium-alloyed weapons, before, during or after detonation.
You may have reason to believe you have been contaminated by uranium under other circumstances including via mining, industrial, manufacturing or other occupational situations.
If you answer "yes" to a number of the questions contained in the self-assessment, we advise you to seek the aid of a physician who is knowledgeable in the diagnosis of internal radiological contamination by uranium isotopes.
You may contact UMRC to arrange a professional assessment or medical referral, or to discuss the appropriateness of participating in UMRC's screening program.
